Add a "feedback" link to each members account, perhaps below the post count. Link it to a single feedback thread for each user. This would also make it easier to post feedback and it would eliminate the chances of multiple threads for any one user account.

Good idea, however it is a very time consuming task to initiate and maintain.
Unless of course there is a PHPBB Custom Mod that adds this to the profiles automatically.
