does anyone ever just replace the clutch and not also the pressure plate and bearing?

It is generally a bad idea to do that. They both take abuse and thus have the possibility to go out around the same time. Why would you want to replace just one?
Also, remember to resurface the flywheel and have it checked for cracks, then balanced if needed.
